Family-MedicineWe believe a healthy community starts in the home – your home and ours. When you receive care at One Community Health, you join the over 10,000 patients in the Columbia River Gorge – babies, children, teens and adults – who make up our medical home.

Our “Patient Centered Medical Home” model ensures that you and your family get the best care possible. Under this system, each patient and/or family will become part of a team with 3-4 providers, medical assistants, a registered nurse, a team assistant, a community health worker, and administrative support staff.

Each time you or your family calls or sees us, you see or speak to the same care team. This allows us to get to know you and your family, helping us give you the specific care, education, and resources you need to be healthy.

People from all over the world and all walks of life live in the Columbia Gorge. To honor that diversity, our staff is respectful of cultural differences. Our goal is to support you through your health care journey by offering the following services: Our primary care medical services include:

  • Comprehensive pediatric care from birth through adolescence
  • Comprehensive medical care for adults and the elderly (geriatrics)
  • Prenatal care at both Health Center sites, with delivery at Providence Hood River Memorial Hospital
  • Preventive services, such as yearly exams for men, women and children, immunizations, screening tests like pap smears and mammograms, and flu shots
  • Family planning services, including oral contraceptives, IUDs and permanent sterilizations
  • Comprehensive pregnancy support services, including education on pregnancy, breastfeeding, pregnancy-related health problems and healthy weight management
  • Most minor surgical services, including: toenail removals, colposcopies, biopsies, endometrial biopsies, contraceptive implants, IUS, wart/skin tag removals, joint injections, and OB ultrasounds.
  • Orthopedic care, including fracture management and sports injuries
  • We also have “packaged” services for uninsured pregnant women and uninsured people with diabetes:
    • If you are uninsured and pregnant, all your prenatal care, including lab work and physical exams, plus your delivery, can be provided at a set, discounted fee. That makes it easy to know which services you need in your pregnancy, and helps make having a baby affordable.
    • If you are uninsured and have diabetes, we offer all of your routine preventive diabetes care as part of a package. The fees are discounted, and you can pay a very low fee each month for a year’s worth of services. These include four visits with your provider to discuss your diabetes and have a foot exam (one every three months), one complete physical exam, two HbA1C lab tests to check your diabetes (one every six months), one Lipid panel lab test to check your cholesterol, one urine test done at our lab to check kidney function, one flu vaccine, and as many Salud Program diabetes education visits as you need. With the package, you know what you need to take care of your diabetes, and we make it affordable.
