Small Changes Can Lead to Big Improvements!

One Community Health is offering a 4-week course covering topics that support diabetes patients in managing their health outcomes. With proper support, diabetes can be managed. Course is offered by the Salud team for all established OCH patients living with diabetes!

What is the Salud Program?

Our Salud program (available in Hood River and The Dalles) provides education and support to patients living with chronic illnesses such as diabetes, hypertension, and obesity. Salud community health workers are part of the medical care team, making sure patients receive the specialized education needed to manage chronic illness. Among other things, we offer support for proper nutrition, exercise, and stress management.

Course Content

Week 1: Diabetes 101 

  • What is diabetes, types of diabetes, why, and treatments

  • A1C and blood sugars – how to check blood sugars, high and low blood sugars, and what the numbers mean

  • Health risks, foot care, and check-ups

  • Treatment and medications

Week 2: Nutrition

  • We will review which foods affect blood sugars, and how to portion those to be beneficial

  • Review nutrition labels and what to look for when shopping for food

  • An in-depth look at nutrition and healthy choices

Week 3: Physical Activity

  • How much physical activity you should do

  • Exercise tips to be successful

  • Safety tips for patients with diabetes and other illnesses

Week 4: Lifestyle Changes

  • Importance of managing stress and diabetes distress

  • Mindfulness and lifestyle changes

  • Sleep deprivation, recommendations and tips
