School-Based Health Services
What are school-based health services?
One Community Health (OCH) believes everyone should have access to health care. That’s why we care for students conveniently where they already are — at school. OCH is partnering with local schools to offer services in Hood River and Wasco counties. Services include medical, preventative, and behavioral health that are offered in schools or on school grounds. School-based health services (SBHS) are intended to increase access to quality health care and lower many of the barriers students commonly face when seeking health care, including cost, transportation, confidentiality concerns, inconvenience, and lack of primary care options. Request an appointment below, or call 541.308.8345.

Sports Physicals
Before your child can play a sport, the state requires a sports physical to be completed by a medical provider. Physicals ensure a childs safety and ability to participate in sports. During these visits, your can also discuss physical and mental health, check their immunization records, and ask any health-related questions. We offer sports physical appointments year-round at all of our clinics, including the School-Based Health Center.
For Hood River County School District students, call/text 541-308-8345 to schedule at the School-Based Health Center at HRVHS.
For Wasco, Klickitat, and neighboring county School District students, call/text 541-386-6380 to schedule at one of our main clinics in Hood River and The Dalles.
On the day of the appointment, don’t forget to bring a Sports Physical form signed by a parent/guardian. Click buttons to download.
Back-To-School Outreach
As part of our school-based outreach services, One Community Health attends Back-To-School registration days, resources fairs, and open houses. We love chatting with families and kids about local health and wellness resources, and providing information about accessing our services to community members. If you’re a family in Hood River or Wasco County schools, look for us at your back-to-school events!
Are you faculty or administration at a local school or youth-focused community organization that would like to request we attend an event? Reach out to us—we’d love to be there!

If you have health insurance we will bill your insurance for the visit. OCH accepts Oregon and Washington Medicaid, Medicare, and commercial insurance plans.
We encourage anyone who is uninsured to apply for Medicaid insurance. If you qualify, this ensures you can get the medical, dental, and behavioral health care you need with minimal cost to you. You may qualify for this type of coverage secondary to your primary coverage. One Community Health has Patient Care Advocates who can help you apply for Medicaid coverage for your family. Call 541-386-6380 extension 20 for bilingual help. Learn more here.
If you do not have health insurance OCH offers a sliding fee discount based on family size and income. Patients will never be turned away because of an inability to pay. Learn more and apply here.

Under Oregon law, minors have a right to consent to their own age-appropriate reproductive healthcare at any age, and can consent to access mental healthcare at age 14 and general medical care at age 15. While confidentiality is a legal right, youth providers know the value of parent/guardian involvement in the health care of their children. Clinical staff talk regularly with youth about how to involve their support systems in medical decision-making, and how to increase parent/child communication. Through conversations with providers and family members, youth are empowered to understand how healthcare systems work and how to access them when they need mental or medical healthcare, which helps build important skills that will support them as they become independent adults. School-based health service providers work hard to center the needs of youth and their loved ones while consistently upholding best practices under Oregon law.
-
All students currently enrolled in the school where a service is being offered are eligible to access that service. This means that any time services are offered at Dufur Schools, for example, any student from the school (K-12) can access the school-based health service. Eligibility is based on school enrollment alone—insurance status and ability to pay are never a factor. Students do not need to already be One Community Health Patients to access school-based health services.
For questions about eligibility, call 541.308.8345.

It is the responsibility of the medical sponsor to keep and maintain confidential medical records, just as you would expect at the hospital or your PCP’s office. These records are subject to federal and state laws regarding access and confidentiality the same as any other medical office.
Hood River Valley High School Health Center Satisfaction Survey:
Students between the ages of 12 and 19 who visit the SBHC may be asked to complete an anonymous survey after their visit. The survey asks about the student’s satisfaction and experience at the SBHC, as well as some general questions about their physical and mental health status. Students can refuse to take the survey and this will not affect their ability to get care at the SBHC. To see a copy of the survey, please visit SBHC Data Requirements. If you have questions about the survey, please email the SBHC State Program Office.
School Exclusion day is February 21, schedule your childs immunizations today.
