Schedule Your Child’s Sports Physical!

Before your child can play a sport, the state requires a Sports Physical to be completed by a medical provider. Sports Physicals help make sure that a kid is safe and able to participate in a sport. They are also a great opportunity for your child to check in with a provider about their physical and mental health, make sure all their immunizations are up-to-date, and get any health-related questions answered. We offer sports physical appointments year-round at all of our clinics, including the School-Based Health Center.

For Wasco, Klickitat, and neighboring county School District students, call/text 541-386-6380 to schedule at one of our main clinics in Hood River and The Dalles.


On the day of the appointment, don’t forget to bring a Sports Physical form signed by a parent/guardian:

Some questions to consider when scheduling your child’s Sports Physical:

  • Has the student had a Sports Physical done at any point in the last two years? If so, it might just be a matter of tracking down that old form through their primary care provider, or wherever they were last seen. Sports Physical forms are valid for two years, and do not need to be re-evaluated every year. Clinics are often happy to simply re-print the forms for prior patients.

  • Has the student had a Well Child Check in the last year? If so, they should contact the provider that did that service, and see if they are willing to sign off on a Sports Physical form. Well Child Checks are similar to Sports Physicals, so if it is semi-recent, many providers may be willing to simply sign off on the form without an extended visit. 

    • This is particularly important because insurance providers usually only cover one Well Child Check each year for a student. If the student has already had a Well Child Check and then comes to get seen by us for a Sports Physical, they will likely be billed by insurance. We definitely want to avoid families getting extra bills!
