Having the correct Primary Care Provider assigned to your OHP plan will help prevent billing problems

Navigating through your Oregon Health Plan (OHP) benefits plan can be complicated. One Community Health wants to make it easier. Having the information you need and the right Primary Care Provider (PCP) associated with your plan gives you the power to make good decisions about your health.

 

How to change or select a new Primary Care Provider (PCP)

 

If the Oregon Health Plan has assigned you to a Primary Care Provider outside of One Community Health, you can request a PCP reassignment to avoid billing issues.

Here are some ways you can change your PCP:

  1. In-person: At any of our clinics, our Welcome Team or a Patient Care Advocates (PCA) can help you fill out the form. PDF-English PDF-Spanish

  • When you are approved for the Oregon Health Plan a primary care provider is assigned to you.

    Your PCP or your assigned doctor is the first doctor you see and the main person who takes care of you. Your PCP works with you to help you stay as healthy as possible. They will also keep track of all your basic and specialty care.

  • You can contact One Community Health in person or by phone at 541-386-6380 and ask for our Patient Care Advocates. They can help you choose or change your PCP.
