Finding Your Official Covid-19 Vaccine Record

You can easily find your COVID-19 Vaccine Record

Once you log in to your MyChart account, click the QR-code symbol (pictured below). The location of the button will be different on the MyChart app and MyChart viewed through your computer browser. Don’t have MyChart? Learn how to access your account here.

You will be taken to a new screen. There you can select whether you would like to view your results as a QR code, downloadable PDF, or download into a different app.
