  • When you arrive at the clinic, please check in at the front desk to let us know you are here. Plan on arriving 15 minutes early so we can verify your information is current.

  • To get address and directions to our clinics CLICK HERE.

Our clinics in Hood River and The Dalles both have parking lots available if you are driving. If you are using public transportation, we have a stop right in front of our clinics. For more information and rates CLICK HERE.

    • Photo identification, such as a driver's license or state photo ID.

    • If you have medical records from other clinics, we can help collect them from other providers. If you haven’t started, please fill out a request for medical records HERE.

    • Cards for any other types of health coverage you may have (like Medicare, Medicaid, or private insurance plan).

    • Current medications including prescriptions, over-the-counter medicines, vitamins, and any herbal supplements.

  • We know that things come up and sometimes you need to cancel or reschedule your visit. Please call or text us at 541-386-6380 as soon as you know and at least 24 hours before your visit, so we can give your visit to another person waiting for the chance to visit with your doctor.

If you miss your appointment without letting the clinic know ahead of time, we'll consider you a "no show" and you'll need to reschedule your appointment.

  • Please call or text us as soon as you can at 541-386-6380. If you arrive late, we may have to reschedule your appointment.
